By providing your mobile phone number to LifeChoice Hospice Palliative Care (“LifeChoice”), you consent to receive periodic SMS or MMS messages from us. These messages may include important updates, service notifications, and other communications related to our hospice and palliative care services.
Providing Your Telephone Number and Notification Obligations
You confirm that the mobile number you provide is accurate and current. You represent that you are the authorized user or owner of the number submitted. If your contact information changes, including your phone number, you agree to notify us promptly at info@mylifechoice.org before the change takes effect.
Consent to Receive Automated Communications
By voluntarily providing your number, you give explicit consent to receive recurring automated or manual text messages from LifeChoice related to our services and your relationship with us. Your consent is not a condition of receiving care. Standard message and data rates may apply.
Opt-Out Instructions
To unsubscribe from SMS or MMS messages at any time, text STOP, END, CANCEL, UNSUBSCRIBE, or QUIT in reply to any message you receive. You may also email us at info@mylifechoice.org to request removal. You may receive a confirmation message confirming your opt-out request.

Carrier Participation
Our SMS service is supported by major U.S. carriers including AT&T, T-Mobile®, Verizon Wireless, Sprint, Boost, U.S. Cellular®, MetroPCS®, InterOp, Cellcom, C Spire Wireless, Cricket, Virgin Mobile, and others. However, delivery is not guaranteed on all carriers. T-Mobile is not liable for delayed or undelivered messages.
Data Collection and Use
In connection with this SMS service, we may collect your mobile phone number, carrier information, message content, date, and time. This information may be used to deliver messages and services you’ve requested.
Service Modifications and Termination
By subscribing to our messaging service, you acknowledge that LifeChoice reserves the right to modify or discontinue the service at any time, with or without notice.
Indemnification
You agree to indemnify and hold harmless LifeChoice against any claims or liabilities arising from the use of a phone number you do not own, failure to notify us of changes to your number, or violations of applicable communication laws such as the Telephone Consumer Protection Act (TCPA).
Participation Requirements
By using our SMS service, you confirm that you are at least 18 years of age, have a mobile device capable of two-way messaging, and subscribe to a wireless service with text messaging capabilities. Service availability and compatibility may vary by carrier and device.
Miscellaneous
You affirm that you have the authority to agree to these terms. If any part of these terms is found to be invalid or unenforceable, the remainder shall remain in full force and effect.
