Specialized Services

Life Choice is dedicated to ensuring every patient receives the specialized care required to ensure a peaceful transition. This requires a multifaceted team of at-home hospice professionals to address your needs.

• Some patients will require more attention from our medical director, who works directly with your physician.
• Some patients will require varying levels of medical and nursing care provided by our RNs and CNAs.
• Some patients will require more socialization and emotional support from our CNAs, volunteers, and bereavement counselors.
• Some patients will require non-denominational support from our Hospice Chaplain.
• Families and patients may have questions and require guidance regarding their condition and care.
• Family caregivers may require respite or assistance coordinating additional community resources.
• Our Social Worker can assist with financial planning, advanced directives, and community resources.
