• Interviews patients, assesses priorities and documents biopsychosocial assessments and progress notes.
• Provides patients and families with education on advance directives and end-of-life care.
• Consults with members of the multi-disciplinary team in liaison with community resources to support the patient’s care.
• Assists patients/families with obtaining community resources.
• Develops and maintains working relationships with community social agencies.
• Performs other duties as assigned.
