Your Role:

As a Hospice RN, you’ll provide compassionate, patient-centered care while coordinating with an interdisciplinary team to ensure patients’ physical, emotional, and spiritual needs are met.

What You’ll Do:
  • Plan, coordinate, and deliver hospice care according to individualized patient care plans.
  • Collaborate with patients, families, and the interdisciplinary team (IDG) to ensure seamless care delivery.
  • Conduct comprehensive assessments and develop care plans tailored to each patient’s needs.
  • Provide education and guidance to patients and families about the dying process and healthcare decisions.
  • Perform and document patient evaluations, visits, and care in compliance with federal, state, and accreditation standards.
  • Monitor patient conditions, report changes to the attending physician, and adjust care plans accordingly.
  • Supervise and evaluate care provided by Hospice Aides and LPNs, ensuring high-quality service delivery.
  • Maintain an up-to-date understanding of hospice regulations, current drug therapies, and best practices.
  • Offer dietary counseling, addressing changes in diet at the end of life.
  • Participate in IDG meetings, QAPI activities, and staff development opportunities.
Qualifications:
  • Graduate of an accredited School of Nursing.
  • Current RN license in IL.
  • Minimum of 1 year of RN experience; hospice experience preferred.
  • Exceptional assessment and problem-solving skills.
  • Strong organizational and time-management abilities.
  • Effective verbal and written communication skills.
  • Licensed driver with a reliable, insured vehicle.
  • Commitment to maintaining patient dignity, safety, and comfort.

Physical Requirements:
  • Ability to perform physical assessments and patient care activities, including lifting, turning, and mobility assistance.
  • Capability to work in various patient environments, including homes that may not be wheelchair accessible.
Job Type: Full-time
Pay: $80,000.00 – $95,000.00 per year
