Initial Evaluation & Clinic Documentation Review

After the referral, our clinical team gathers the necessary medical history and records. We coordinate with doctors, hospitals, and other care providers to confirm that hospice is the right fit — both medically and emotionally. Insurance Verification

Nurse Assessment Scheduling

After our initial conversation, a LifeChoice nurse will schedule an in-home (or facility) visit.
